Medical billing, credentialing and AR recovery

Get paid for every claim you’ve earned.

MHB helps practices collect more, faster. We enroll providers correctly, submit claims clean, and chase every denial until it’s resolved.

The problem

Every practice has a leak. Most never see it.

11.8%

average first-pass claim denial rate across U.S. practices

10–16%

typical denial rate in behavioral health billing

2–5%

of net patient revenue is written off permanently (HFMA benchmark)

~90%

of denials are classified as avoidable

How it works

One system for the three places revenue disappears.

01

Enroll and credential

Every provider credentialed and enrolled for electronic claims with every payer, so claims can actually be paid.

02

Submit clean

Coding reviewed and claims scrubbed. Rejections are caught at the clearinghouse before they become lost revenue.

03

Recover and report

Denials appealed line by line, aged AR worked at 30, 60 and 90+ days, and reporting you can actually read.

Services

Everything from enrollment to collections.

Medical billing and RCM

Charge entry, claims filing, payment posting, patient statements and fee schedule optimization.

Credentialing and enrollment

Group and individual enrollment, multi-state setups, CAQH and NPPES upkeep, and revalidations.

AR recovery and audits

30/60/90+ day AR cleanup, line-item denial appeals and root-cause rejection analysis.

EDI, ERA and EFT setup

Electronic claims connectivity, remittance and electronic payment enrollment.

Group and network support

Group entity setup, individual-to-group transitions and clinician roster additions.

Dedicated virtual staff

Billers, AR and intake specialists working as an extension of your team.

Specialties

Built for behavioral health and SUD practices.

Behavioral health runs higher denial rates and more payer complexity than most specialties. We know the Medicaid, MCO and commercial rules that trip practices up.

What we catch
Not a billing problem. An enrollment problem.
Symptom
Claims rejected across several payers while credentialing looked complete.
Root cause
The provider’s NPI was never enrolled for electronic claims through the clearinghouse, so claims bounced before reaching payers.
Resolution
Complete transaction enrollment payer by payer, then re-run the rejected claims.

Why MHB

Traditional billing vs. MHB.

Feature
Traditional vendors
MHB
Billing fees
Flat monthly fees or passive percentages
Tied to what you collect
Denials
Worked occasionally or written off to aging AR

Your team
Anonymous call center pools
A dedicated certified biller
Credentialing
A costly, separate add-on
Integrated with billing to prevent claim blocks
The economics
$124,150

per year for a lean in-house team plus the denials it still can’t recover: $100,750 in loaded staffing and $23,400 written off.

Illustrative 3-provider behavioral health practice: 1 biller/coder plus 0.5 FTE credentialing at 1.3x loaded salary (BLS); 13% denial rate and 3% write-off of about $780K net revenue (MGMA/HFMA).

FAQ

Questions, answered.

How does MHB get paid?

Billing fees are a percentage of what we collect, so we earn when you do. Credentialing and dedicated staffing are flat fees. Everything is confirmed in writing before you start.

Do I have to change my software or billing company?

No. We work with your existing practice management system and clearinghouse using scoped access, and we’re happy to act as a second set of eyes alongside an existing team.

What does the free trial involve?

A seven-day review where we actively work a batch of your rejected, denied and aging claims, then deliver a written audit of denial trends, coding errors and enrollment mismatches. No cost and no contract.

How is patient data protected?

A signed data-handling agreement is in place before any patient information is shared. Access is limited to billing work, time-boxed, and handled in a HIPAA-compliant way.

How long does credentialing take?

It varies by payer, typically anywhere from about 45 to 120 days. We give you a clearer estimate once the provider and group structure are set.

Do you work with groups and clinical networks?

Yes. We handle group enrollment, individual-to-group transitions and roster additions, and offer co-branded or white-label partnerships.
